Abstract

NUR AFNI RINDIANI, Review of The Patient's Medical Record Numbering System More From One At BP Cilandak Clinic. Scientific Paper, Jakarta: D-III Program of the Bhumi Husada Jakarta Medical Recorder and Health Information Academy in 2022. The purpose of the unit numbering system is to make the entire patient's treatment episode easy to trace and complete describes the patient's sick/health history. Based on preliminary observations at the BP Cilandak Clinic using a unit numbering system, but it was still found to provide more than one medical record number. This may result in unsustainable information. This study aims to determine the system of providing medical record numbers at the BP Cilandak Clinic. The research method used is descriptive research, with the population of all new patients in 2021 totaling 1,548, and samples taken using the Slovin formula totaling 318. Based on the results of the study from 318 samples, 62 patients (19.5%) had more than one medical record number. Factors causing the numbering of medical records more than one are that there is no medical record numbering system policy used, no Standard Operating Procedures for handling more than one medical record number, the qualifications of officers are not appropriate, and the database created is incomplete and there are errors in typing patient list reports. In this study, the authors suggest creating a medical record numbering policy, Standard Operating Procedures Handling more than one medical record number, merging databases and physical medical records that have more than one number periodically into one number, providing socialization to officers about the numbering system, and adding patient date of birth items to the database.
